Actually F2 race cars adhere to the 2005 FIA F1 safety standards. They have wheel tethers, just like F1 machines. Unfortunately, the cable was not able to witshtand the forces of the crash and the wheel flew off killing poor Henry. A freak accident indeed, but one that shows that motor racing is dangerous irrespectively of the major strides made on the safety front.
RIP John Surtees. A tragic day indeed for motorsport.
PS: Actually, this has been a disastrous weekend for racing in general. On Saturday italian Flavio Guglielmini, aged 37 - co-driver to Brian Lavio, died tragically when the Peugeot 207 S2000 rally car they were competing with in the Bulgarian IRC Rally went off the road and crashed into a tree. RIP Flavio.

I didn't know that. Any reason was given as to why the wheel flew off?

OMG that is horrifying to watch. If you look closely, it is not just the wheel that hit him, it is the inside of the wheel, with suspension components still attached! That is terrible. Racing remains a dangerous sport!
RIP Henry
